(I)LOST THEN APPLY
1. A letter to the Embassy on the matter.
2. Copy of the lost passport
3. FEE: 1150.00
4. Police Abstract
5. Two passport size photographs.
(II) FOR BABY
1. A Letter from the parents to apply for a new passport
2. Copy of the Father and Mother’s passports
3. Copy of the Parent’s Certificate of Marriage.
4. Copy of the Birth Certificate
5. Two passport size photographs.
6. FEE: depend on how many pages they applied ( RMB450 for 48 pages; Rmb600 for 64 pages)
